REQUIREMENTS

The nomination package submitted for the Global Achievement Awards must contain the following: 

• Completed nomination form - please be sure to complete all required sections and save your work. You may return and finish your submission at any time before the submission deadline. 
• Include one to two sentence citation (150 words or less) summarizing why the person should receive the award category. Include high-level accomplishments of the candidate based on the criteria established for this award. 

Optional: 

The following items are optional, but will help strengthen your nomination submission:

• A curriculum vitae or LinkedIn profile link for the nominee (non-prescriptive). Include the candidate’s name, address and email, history of employment, degrees, research experience, honors, memberships, and service to the community through committee work, advisory boards, etc. 
• A recommendation letter (300 words or less). Letterhead stationery is required. Nominator’s name, title, institution, and contact information (phone number and email address) are required, along with a signature (digital is acceptable). The nomination letter should account for the accomplishments of the candidate based on the criteria established for this award.  The citation should appear at either the beginning or end of the nomination letter.
